for the NHS has a legal duty to keep

except in exceptional cases (usually
when the health and safety of others
is at risk). There are interview
rooms at the surgery for use if you
need to speak to a member of staff
about a sensitive issue.
The practice will
not tolerate threatening, violent or
abusive conduct towards any of its
personnel, patients or others
present on the practice premises.
Any such act could result in police
involvement and removal from the
list of registered patients.
The practice uses a computer
system to store medical details.
This information is secure and
cannot be viewed by any unauthorised
person. You are entitled to see the
records this practice keeps about
you if you wish, applications to see
your medical records should be made
in writing.
Health professionals need to share
essential, relevant medical
information to provide the best care
for the patient. Medical information
may also be used for research or
clinical audit in an anonymised
form. If you wish to withhold your
consent please advise your GP.
