When you register you will be registered with the practice as a non limited partnership. We will offer you a named partner on registration, and you will be offered the doctor of your choice whenever possible.

If you wish to register with the practice and live in the practice area, please request a registration form from reception. Or you can open and print a copy of the GMS1 Registration form by clicking here.

Please also complete and return the new patient health questionnaire which you can download here. This information is important to your new GP when you register.

On registering, you will need to show us two forms of identification. One can be a passport, driving licence, birth certificate etc. The second must be proof of your current address. This could be a utility bill, rent book or bank statement.

If you are registering a child aged under 5 years, please complete the Health Visitor Registration form, which you can download here, and return it to the surgery with the registration forms.
 

We encourage our patients to try to see one or two regular doctors for any ongoing medical condition to maintain continuity. This need not be the doctor with whom you are registered.
