The information we collect about you through the form on the practice website is to allow us to contact you periodically by email or SMS/Text messaging.
You can remove yourself from this email list by unsubscribing the email address you first entered in the form below.
If you are unable to do this please send a written request to the Practice Manager with the email address and your name and this can be done manually on your behalf.

All the email newsletters that you are sent have an option to automatically remove your email address (unsubscribe).

Your data is stored securely on our secure servers based in the UK. It is not stored within the NHS.
Your email address is not shared with ANYONE outside the Practice or used for ANY other purpose.
